Dear Parents/Caregivers,

If your child is doing an activity before or after school (eg. Basketball, Football, Cross Country etc), whilst they are also booked into OSHC, you must sign and complete a Release Form with OSHC, with your child’s name, Activity, Day and Times.

These forms are available from OSHC.
